Maryland Hospital Credentialing Application
In 2002, the Maryland General Assembly passed SB 819-
Hospitals- Uniform Standard Credentialing Form. The bill
required the Department, in conjunction with stakeholders,
to develop an application for the initial credentialing of
health care practitioners to Maryland hospitals.
