Q. How do I apply for an application to become a licensed or Medicare provider under the Ambulatory Care Licensure Program?

A. Application packets may be obtained by forwarding a request in writing, by mail, or by fax. A pending file will be developed to begin the application process. Q. What are ambulatory care facilities under the Ambulatory Care Licensure Program?

A. Ambulatory Care Programs include: Ambulatory Surgery Centers (ASC), Birthing Centers, End-Stage Renal Dialysis Centers (ESRD), Comprehensive Outpatient Rehabilitation Facilities (CORF), Home Health Agencies (HHA), Hospice Care Programs, Outpatient Physical Therapy (OPT), Major Medical Equipment (MME), Portable X-Ray, Residential Service Agencies (RSA).
