Chapter 20 Fees

 Authority: Health Occupations Article, §§ 4-207 and 4-304, Annotated Code of Maryland

A. A person shall pay a fee as set forth in Regulation .02 of this chapter by check or money order.

B. The Board shall provide a copy of this chapter:

(1) Along with any application form or renewal form and

(2) To any person on request.

C. The Board shall refund the initial licensure or renewal fee, less a $50 processing fee, to a dentist or a dental hygienist who does not qualify for initial licensure or renewal.

The following fees are established by the Board:

A. Initial Application fee:

(1) Dentist.................................................................................	$450

B. Limited license fee:

(1) Dentist.................................................................................	$225
(2) Credentials processing (foreign dental school graduate      surcharge)............................................................................	$75

C. Teacher’s license fee:

(2)Dental hygienist.....................................................................	$225
(3) Credentials processing (foreign school graduate surcharge).......	$75

E. License renewal fee:

(4) Dentist late renewal fee (July 1 through July 31 of the year in      which the renewal is due).......................................................	$150

F. Dental radiation technologist certification fee:
