A. A person shall pay a fee as set forth in
Regulation .02 of this chapter by check or money order.
B. The Board shall provide a copy of this
chapter:
(1) Along with any application form or
renewal form and
(2) To any person on request.
C. The Board shall refund the initial
licensure or renewal fee, less a $50 processing fee, to a
dentist or a dental hygienist who does not qualify for initial
licensure or renewal.
