How do I get a Counselor Number?

Every person who wishes to be a CTR Test Counselor at a Maryland AIDS Administration testing site must attend a four-day training (for exceptions to this rule, call (410)767-5018 and ask to speak to a CTR Coordinator). Upon completing the training, a Counselor Number Request Form should be completed and faxed to (410)333-4805. The Counselor Number Request Form can be found in the HIV CTR Testing Manual in Chapter 1.

The HIV CTR Testing Manual has information about PTC by Telephone in Chapter 7. Additionally, you can call (410)767-5018 and ask for a CTR Coordinator or ask to speak to a Training Specialist.

How do I become a testing site?

To become a CTR Testing Site with the Maryland AIDS Administration you should contact a CTR Coordinator. They will send you a packet of information and help guide you through the application process. Call (410)767-5018 and ask to speak to a CTR Coordinator.

How do I get a Site Number?

Every organization that wishes to be a CTR Testing Site under the MD AIDS Administration must submit an application. Once an organization has been approved to provide testing, a Site Number Request Form can be completed and faxed to (410)333-4805. The Site Number Request Form can be found in the HIV CTR Testing Manual in Chapter 1. If an organization is already approved to be a CTR Testing Site and would like to add an additional site number, simply fill out the Site Number Request Form and fax it to the number above.

Where do I find the Country, State, County or Language Codes (also called FIPS Codes)?

FIPS Codes can be found in the HIV CTR Testing Manual and on the counselors laminated sheet. If you cannot find a code that you need, please call (410)767-5018 and ask for a CTR Coordinator.
